THE EFFECTS OF PENTOXIFYLLINE AND DEXAMETHASONE ON COMPOSITE GRAFT SURVIVAL



Abstract
The composite grafts have been used in reconstruction of the defects on nose, nipple and finger tip etc. But the use of composite graft in facial reconstruction has been limited by unpredictable survival rate beyond I cm of radius.
In order to enhance survival rate of the composite graft, many clinical trials were made. Numerous drugs were employed to improve the survival of cutaneous and myocutaneous flaps. But any agents have not been studied in composite graft.
Pentoxifylline, which is known as a vasoactive agent for cerebral vascular insufficiency, increase graft survival by its ability to increase erythrocyte deformability and of septic and circulatory shock. The effect of steroid on improvement of vascular perfusion can be attributed to the prevention of platelet aggregation and to stabilization of lysosomal membrane.
Using rabbit model, full-thickness auricular composite grafts were made and reattached. The animals were received intramuscular dexamethasone and intravenous pentoxifylline daily during the first 7 postoperative days each other. At 14 days the survival areas were assessed and the grafts were biopsied on the 1st. 7th and 14th days for histopathologic examination
It was observed that pentoxifylline and dexametasone treated groups has significant effect on the graft survival when compared with the control. In histopathologic results, the pentoxifylline group revealed more tissue edema and the dexamethasone group showed lower inflammatory reaction.
